Two more changes are needed to the patient demographics for the NA2013 connectathon

(1) A few more patients need to be marked as "unused: Please change these two keywords to 'unused':
-- Preload PIX/PDQ/PDO
-- SWF/MIMA
-- Preload XDS.b Registry Dashboard

(2) A few of the imported patients from 2012 are missing the value for the "State" field (it must have been like this for Bern!). I have attached a spreadhsheet. The missing values are in Red font. Could those be added??
