GMM: Cannot leave ports blank when adding default config for DICOM SCU

When trying to add a new default config for the Modality actor in the REM-NM profile, I don't want to specify a port number for a DICOM SCU config for the STORAGE SOP Class.

However, when I try to delete the default values, they re-appear.

Also, while you are fixing that dialog box, you should display the Actor name rather than the database index.
