NA2018: Exception when adding 2 monitor user account -- account not added

      Description

      I was adding new accounts for monitors. I successfully added two accounts.

      Then, the last two accounts failed.

      I added the user account details in the 'Monitor' company. When I saved the account information, two messages briefly appeared at the top of the screen.

      -- One message indicated that an email had been sent to the new user.
      -- A second message showed an "Exception", but the message flashed too fast for me to read it.

      When I went to find the accounts, they did not exist (but I suspect the 2 monitors got an email saying they have a new account).

      The two users that had problems are:
      -- username=nriley email=nriley@metrohealth.org
      -- username=eschaefer email= Eschaefer@Sequoiaproject.org
