[CH:PIX/PDQ/XCPD] issues encountered in CH validators

      Description

      Some corrections to brought to the validation tool for EPD HL7v3 messages:

      - Review validator for XCPD query, reports returned by IHE part of the validator which might be confusing (regarding the cardinality of the query parameters)
      - ch_xcpd_005_LivingSubjectId => typo in the description of the constraint
      - ch_pdq_008: should be downgraded to INFO + not all names known for a patient are birth name, I would rather check that at least one name has the BR qualifier
      - ch_pdq_006 and ch_pdq_012 => we are now able to write a constraint to check whether the ID is formatted as an EPR-PID or not
      - review description of CH-PIX-* constraints. The first 7 ones relate to a PIX feed message not a PDQ response
      - CHPIXFEEDPackValidator: it seems constraints are only applied in the case of Add record message. They might need to be duplicated for the Revise record message (it is not exactly the same patient class).
