Review and fix DICOM SCP configuration page

This is the page used by the users to register their DICOM SCP systems (for RAD-5 testing).
Review the schema used to store such configuration (might be interesting to extent the SystemConfiguration class from simulator-common) and update the layout of the page to match the new style
