Change namespace in policyQuery Mock Failure response

According to the specifications, the response should embed a "urn:oasis:names:tc:SAML:2.0:protocol" namespace for the "Response" tag and a "urn:oasis:names:tc:SAML:2.0:assertion" for the inner tags.

PPQ Simulator answers only with the samlp namespace for all the tags.

This discrepancy blocks Bints for their tests (see e-mail attached).
